Arthroscopy Rehabilitation Protocol


Phase I: Beginning day of surgery

Goals:
Decrease Swelling
Increase Range of Motion
Initiate Strengthening
  1. Apply ice 15 to 20 minutes, 3 to 5 times per day until swelling is diminished.
  2. Wear ace wrap until selling is insignificant.
  3. Elevate leg when not ambulatory until swelling is insignificant.
  4. Use crutches, beginning with partial weight bearing, and progressing to full weight bearing without crutches when gait is normal.
  5. If range of motion (ROM) is restricted in flexion extension, do passive ROM stretches, heel slides, wall slides, and heel hangs. (Hold position 20 seconds, and repeat for 5 minutes. Do three sets with 1 minute rest between sets.) Discontinue when active ROM is regained through functional range (0 -120).
  6. Do active ROM exercises while sitting, prone, and 2 times per day. Discontinue when ROM is normal (0 - 140).
  7. Do quad sets, two sets of 20, two times per day until quad tone and bulk are equal to unoperated leg.
  8. Do hamstring sets, two sets of 20, two times per day until quad tome and bulk are equal to unoperated leg.
  9. Do straight leg raises in flexion, abduction, adduction, and extension, two sets of 20, two times per day until returned to full, unrestricted activity.
Phase II: To be started after 1 week post-op visit.

Goals:
Continue Strengthening
Return to Functional Activity
  1. When active knee range if motion reaches 90º flexion, begin progressive-resistive knee extension exercise (PRE). The goal of PRE is to achieve strength equal to the normal strength of 33% of body weight plus 10 lbs.
  2. when active knee ROM reaches 100º flexion, begin cycling for aerobic conditioning and to enhance ROM. Start with 10 minutes of cycling, and add minutes per week.
  3. When weight bearing without effusion or pain, quad extension exceeds 30 lbs an hamstring flexion exceeds 20 lbs, begin step exercises: start with 4" to 6" steps. Do as many step ups as can be accomplished, up to 200. When goal of 200 is achieved, add 1" to step height.
  4. Wall sits - three reps, 15 seconds each.
